All Athletes Register Online!                                            TRYOUTS START NOVEMBER 4, 2024

1. Go to REGISTER MY ATHLETE and create an online account or use the account log in from last school year.

2. Complete online registration for each of your athletes.

3. Be sure to select the current school year.  Include all sports that your child is registering for.

4. Complete all the pages of the physical packet. Physicals for the 2024-2025 school year must be completed after March 1, 2024 and on the current 2024-2025 AIA forms.

5. Complete Brainbook Concussion & Opioid Education courses at ACADEMY AZPREP365.

6. Load the completed physical, insurance card and Brainbook Concussion and Opioid Education certificate to REGISTER MY ATHLETE.

7. All registration is electronic and MUST be properly loaded into REGISTER MY ATHLETE
